divabride has GREAT questionnaires for this type of thing. She definitely has a website where the info is, I'll see if I can find it, but if not, FM her - she'll totally help you out.
ETS: Found it! http://www.freewebs.com/bridaldiva/
She just posted a thread about this last week - I'll bump that for you as well.

Make sure you get EVERYTHING in writing. Ask how many servers there will be per table and how many bartenders per guests.

Re: Questions to ask when you're booking a Reception
I know this is gonna sound silly but ask how old the staff is. You don't want kids serving your guests who would rather be on their cell phones or horsing around with each other. This went on at my FBIL wedding in May and it drove everyone nuts. The girl actually got a piggy back ride through the room by another waiter and stood in the doorway on her cell. They never cleared the table or ashtrays. I would also ask what happens if your guests arrive early for some reason, is there somewhere for them to go besides the cocktail hour opening 15min. early(for those guests that never show to the church and go right to the reception). I'd also ask about food tastings and refunds of your deposit if you don't like their food. If possible ask to see their kitchen too while they are preparing for a party.
