I had my marriage certificate & 2 registration form sent to me by the Babylon Town Hall, but they laminated my marriage certificate. Did they do this for you?
When I went to SS she said that makes the form invalid.

The town hall on surise hwy in lindenhurst, right? My marriage license was definitely NOT laminated. But what registration form are you talking about? I only received the certificate.

Yes the one on Sunrise.
I got 2 NYS Dept of Health Certificate of Marriage Registration Forms w/ a blue border
& then 1 Laminated
State of NY Affidavit, License & Certificate of Marriage
