changing last name?

I posted on the BHB board but I am not getting any responses so I was wondering....
for those who changed their last names, how do I go about doing this?
Where do I need to go?
what do I need to bring?
And what are all the things I have to change?

I'm in the process of doing this now (I wanted to do it sooner, but my marriage license was wrong, so I needed to correct it)

I would first change your Social Security (because everything is linked to your SS#)and then your Driver's License and then from there you could change your Credit Cards, Bank Accounts, at work your: Benefits, Payroll check - W2, etc.
