No charge for changing name; just go to their websites and you can download the forms:
ny.dmv.org www.ssa.gov

I am using a website service to assist with my name change (they are VERY THOROUGH!!!), as I have been procrastinating and there are so many things I would forget if I did not use them... Well worth the $29.95!
