Changing name on passport

Hi ladies,

Just when I thought I changed my name on everything, I come across my passport and guess what? I never changed my name on this.

I printed out Form DS-19 as required by the US Passport Amendment/Validation office. They also want proof of a valid marriage certificate and my original passport.

Now it says no copies. Normally, when I changed my name I was able to send a copy of the marriage certificate, but it looks like they want the original. Am I right?

Thanks ladies!

Re: Changing name on passport

yup- you have to send in your original passport and original marriage certificate. I hate sending in my original marriage certificate but that's their requirements. But on a good note, I have heard from many people that you get it back. According to the post office, you get back your original passport back with an addendum in the back with your new name. I am planning to do this once I am done with all my other offical name changes.

Yes...this is exactly what happens. I sent away my passport and my original marriage certificate....I got them back about 4 weeks later....with an amendment on the last page with my new name.

Re: Changing name on passport

I am going down tomorrow to town hall to do all of this. I am going to see if I can get an origonal copy to send to the passport place , they are right next to each other in the building. Then I am going to go to motor vehicles next week. UGH! men are so lucky.

Re: Changing name on passport

When you say that you have to send your original marriage certificate, you do mean a certified copy of the original, right? I don't see why you would ever have to send anyone your real original. A 'real' copy, not a photocopy, should do. Or am I wrong? I need to change my name on my passport too!

Re: Changing name on passport

You have to send a certified copy of your marriage certificate and your current passport. Here's the info...

Re: Changing name on passport

It says to send 'a certified copy' which is NOT the original. You can send the original if you want, but I wouldn't! You can get certified copies of your original from the town office where you got your original from. You have to pay for the copies, but it's not much money. They have the town seal pressed into them, making them certified copies.

Re: Changing name on passport

I sent in my original and got it back....I dont see what the big deal is, if it got lost, I would have gotten another one from the town.

Re: Changing name on passport

OMG I thought I had to get a new picture, fill out all those forms...thankfully I don't!

I am however, going to pay for the expedited service. I don't need it until November but all the same I'd rather like to get it back asap.

Thank you!
