okay so we've been married for like a month and 1/2 and i still haven't gotten around to doing any changes. i know i need to go to the DMV i guess to change my last name? what do i have to bring w/ me and is that the only way i need to change my legal last name??? (sorry if thats confusing) lol

First start off at the social security

You need to bring your marriage certificate and then your old social security card.

For the DMV all you need is marriage certificate and fill out form MV 82 and it cost $10. they will make copy of your certificate

I mailed mine in for social security since i couldn't take a day off work. It took awhile, but it was worth it. I used the SS-5 form and sent it in with my marriage certificate.

The DMV wasn't bad, just the marriage certificate and the MV 44 form. I forgot how much it was.

The worst for me was my banks.

I don't think SS even asked me for my old card. They looked at the marriage certificate. Bring identification.

I did my SS card last week and I didn't have my old one with me. You just have to fill out the form and bring your marriage certificate. You don't even need ID i don't think (but i had my driver's license with me). It was 1 hour of waiting, and 2 minutes with the lady to do the change. Oh, maybe 3 minutes... she spelled my new name wrong the first time and I refused to sign it until she fixed it!

for the dmv all you need is your id and marriage license and the m44 form filled out

social security you need baptism/marriage/id

I have to go to the DMV too this week..not looking forward to it

You def. dont need your old SS card to change your last name there... I went today and she asked me for it and I said it was at home and she said just to destroy it.

All you need is your marriage certificate and a form you can print online


At the DMV you need your license and your marriage certificate and that form.

I brought my passport just in case to both places, but neither of them asked for it.

I didnt need mine, but I did bring my passport and drivers license as ID. I think I actually only showed them my drivers license
