i never knew it was there till a few weeks ago but i cant find it online. Did anyone go there to change your name?? what did they make you show as ID? i found some forms online but its asking for liek 4 forms of ID and both of my parents SSNs and stuff i dont think its the right one. TIA

I did. I brought my marriage license, my old SS card, my driver's license, my birth certificate just to be safe. I went early on a Tuesday during a school vacation. I didn't bring a form. I think I filled it out there. Honestly, I can't remember what I did when I got there, but it wasn't bad.

see i cant find my BC but the form i found said they wont take it as proof anyway that i needed a passport or something lol maybe i messed up.....i cant even find a number for them to call and ask if my old ssn license and marriage certificate is enough,...,

i went all i did was print the forms off the website, filled them out (including all my parents info its required) and brought my drivers license, my ss card and marriage license. thats all you need. it goes quicker if you fill out the forms in advance and besides your not going to know your parents info there. Go on a friday after 1, thats what they told me when I called to make an appointment (which they dont take appointments)
Its really very easy.
Be warned they take you SS card and you wont have one for aprox 2 weeks till it comes in the mail, so if your going to need it for anything, be prepared now.
