Help me, please?

Do u guys know how I would go about changing my name on my passport? I would like to do it by mail, if possible. Do I need to order a form, where can I order it, and what do I need to send?

I just mailed mine in. I printed out this form Form and mailed it in.

I wanted to bump this up. I just got my new SS card in the mail and now need to get a new passport - we're going to Mexico in 3 weeks!!

Bring your marriage license with you. That will do it. I don't think that 3 weeks is enough time and it's really not worth the rush fee (if they can do it.)

We went to Europe for our HM. On the way back into the country we were talking to the customs officer. He said not to bother changing my passport. That they would charge me money and then just type the name in the back. He said to carry my marriage license with me and that would do and to change my name on it when it expires (mine expires this year.)

I called the passport office here and if you go in person they'll do it right away (the catch is that you must be traveling within 2 weeks).

ETA: And, there is no charge to have your name changed if its due to marriage and do it through the mail.
