Just to clarify, MAILING SS question..

BEANS' thread

ok...so based on that and the fact that after 90 minutes today i was told it would be HOURS until my number was called to simply change my last name on my SS card, I decided to mail it in. I know WHERE to mail it...but WHAT do I send in?

OBVIOUSLY the following:

-marriage certificate
-application for change/new card

WHAT ELSE?

TIA.

I will give my 2 cents.

I went into a local S office, just off Rte 110 in Huntington, arrived at 9:01 was out of there by 9:20.

A co-worker who was married May 25, 1 month before me, mailed in her paperwork to SS w/i days of receiving her license (which was about 3 weeks post wedding). She never rec'd it, so called last week (at least 3 months after the wedding) and spent 90 minutes on the phone and finally got a person who had literally JUST processed her paperwork just before she called.

I went today and was able to change my last name on my ss card. The only thing they asked for was my old ss card and marriage certificate.
I will get my new card in the mail within 2 weeks.
I only waited 1 hour in the Patchogue office.
