First of all, did you agree to be in her bridal party before or after she did these things to you? If your feelings are that strong, maybe you should pull out and then you would not have any responsibilities.
But, if you still wish to continue being in her bridal party, I would suggest doing your share with the shower (not fair to others working, regardless of bride's obnoxious behavior) and opting out of a group gift for a smaller present.

Mary, I don't want to pull out of the BP we have been friends for over 10 years, just this crap is bothering me, and what was she thinking after she told her cousin to call me to plan this shower. I think helping and giving a smaller gift is best to do.

Honestly, I think you have a lot of pent up resentment against her (for good reason) and as a result, you SHOULD NOT be in her bridal party.
I think sometimes that people don't know what goes into showers and weddings until they are the bride...
I know my BP was a mess, b/c my sister never contacted anyone b/c she didn't like one of my friends...(b/c she suggested we go away for a bach party...)
I know that she didn't do much, but I know when she gets married that I will go all out for her. mainly b/c I know she didn't know what is involved...
I say take the high road...my sister made flyers for my shower invites...and while I could have done something as equally not nice...I decided to make some fabulous invites...
