Name Change Process

Name Change Process

I'm sure this topic comes up constantly here...
If I'm reading the documents that were sent with my marriage certificate right, then it looks as though I need to do name changes IN PERSON? Rumor has it, I need to do SSN, and then DMV?

If someone can advise, or bump an old thread it would be appreciated.

Thanks Newlies!

Social Security and DMV must be done in person. Make sure you bring your marriage license with you. I don't think it matters what order you do it in. I changed my name at DMV first and the Social Security.

i just bumped the thread for you...

i did SS, then DMV, then the bank in one day in that order... I did hear that you can do SS through the mail, but then you have to mail in your orignal marriage liscence, which made me nervous...

Can you download the forms, so that you have everything ready when you get there?

I downloaded the form online here:
http://www.socialsecurity.gov/online/ss-5.pdf

but do I really need my parents SS#'s to complete this?

Nevermind...I read on some old threads that it is not necessary even though it is on the form.

I went to the one in Patchogue. They send your card in the mail, takes about 7-10 days.

I did it in this order:

1. Bank-Made all our accounts into joint with my new last name and ordered new checks
2. Went to SS office (you can find one located nearest to you on the internet)
3. DMV-Change of License
4. Work-Change of Payroll
5. Grad school records
6. Student Loans
7. Credit Cards
8. Car Insurance
9. Passport

I was at the one in Freeport today. It's on Sunrise Highway. They said it will take about 5 days to get the new card in the mail
