Re: Name Change- SS# Dept.
You'll have to leave the original mariage license with them. They don't do it right there. I just got my SS card in the mail. I had to mail them my oringial marriage license then they sent me my card.As for the DMV, I just brought my marriage license to show them my name change (I'd make a copy before you give it to SS). I took a new picture and they gave me a temporary one.
Good Luck

I would go to the DMV first (I showed them my original) and then submit your original license to the SS office. I'm not sure if the DMV takes a copy, you can call and find out.

I went my my original marriage liscence to SS office and they made a copy of it. The DMV did not make a copy they just had to see it.

Re: Name Change- SS# Dept.
Do you need your original social security card at the SS office or just the marraige license? Do you need photo ID as well?

Re: Name Change- SS# Dept.
I went to the SS office in Patchogue - I was in and out in about 10 minutes. All the needed was proof of your old name (drivers license) and then your marriage certificate. They made a copy of the marriage certificate, I also had to fill out a form. And they mailed me my new SS card (I received it exactly a week later).I then went to the DMV in Medford (in and out in 5 minutes), had to take a new picture
and show them my marriage certificate. I received the new one in about 1 1/2 weeks later (with my crappy picture
)

Re: Name Change- SS# Dept.
I already changed it at DMV, now I have to find my license with my maiden name, I thought I kept them together.

Re: Name Change- SS# Dept.
I went to the SS office and filled out an application, showed them my marriage certificate and that was it... they didn't make a copy of anything... and gave me a print out showing the new card would come in about 10-12 days... it took about 5 mintes to do...very easy...Welcome New Vendors
