Has anyone recently done this.. Do you fill out the form with your new name im going nuts....theres no section that asks you for your new changed name., just asks you for your old name.

jsut an FYI:
if you dont feel like doing it - you dont have to
make sure that you book all flights in your maiden name and you can keep your passport with your maiden name
i am actually going to just wait until i have to renew it and then apply with my new name
since i didnt do it myself i am sorry i couldnt be more helpful!!

you are going to put your new name on the application.
then where it says 'have you ever used another name?' you enter your maiden name in box. 9
ETA: if your passport has been issued less than a year ago--there is a different form to fill out and no fees
and like Tilde said--you don't HAVE to change the passport. i don't plan on changing mine until i have to renew in about 5 years!

I'm keeping mine in my Maiden name also.. they charge you $75 to change the name and it still expires whenever your old one expires...which to me is a RIP OFF!!!

oh god d a m n it....I JUST told my TA today when she asked me if I wanted to use my maiden or married name on the tickets for our Europe trip....I told her I still have to change my passport to my married name but that I'm going to, so to put the tickets in my married name!!!
I thought I had to b/c my license has my new last name!!!
grrrrrr!!!!!!!!
p.s. I can't find that form either....you weren't alone. i tried to do it last month and got so annoyed that I couldn't find it...that I was like FORGET IT! haha

Knowing that I just got mine about 2 years ago and it costs $75 to just change the name I am just keeping it the way it is until I need to - then I might just book under my maiden name for the time being.
This is a RIP OFF!

I booked our summer cruise and flights under my maiden name. My passport expires in 09 so I'll just change my name when I renew. I still have a NYS drivers license in my maiden name too.

Question for you experts, on a similar topic:
I changed my SSN number officially last week. I don't want to change my liscense yet because we are planning on moving in 1 year.
Anyone know if its bad that I have no photo ID in my new name since I already changed my SSN?
Really asking cuz we are going to be purchasing an apt and I want the apt and morgatage to be in my new name not my old one.
I was planning on changing my passport but now that I read this post, I am considering waiting...

If that's the case you might want to change at least one piece of ID like a passport or license to your new name.
I just got done checking a few places off the list- going to get my SS card done on Monday
