Quick question about name change

Hi girls, I hope you can help me with this.

I went to the Hempstead Town Hall to get my marriage license. And i originally wanted to switch my maiden name to be my middle name and take on FH's last name (I didn't want to hyphenate). But when I told the lady that, she said she doesn't do middle name changes and would only change my last name to his.

Can I still add my maiden name as my middle name if it's not on the marriage license?
