thanks girls, i printed out the form and it had a space for eye exam i got scared lol i'm not good at those

I didn't, but I had to take my picture again. I was in and out really quick though. I printed the form on-line and had it filled out already.

I didn't. They made me take a new picture though (when changing your name they require a new picture)...I wish I had known that before going...I was dressed very casually and DH and I were on our way to go to yard work....I wish I had on more makeup and did my hair lol Oh well.
