Re: Step Parents
I think you should take your cue from your DH. Mine has a stepmother too, but he's definitely not close with her, so I tend to introduce her (not that I have to very often, it was just at the wedding, I think) I would introduce my FIL first and then 'and his wife Alison'...
I actually asked DH recently when we have kids what should they call her. He thinks just her first name...but I would like her to be included and be called 'grandma alison' or something. I mean when you think about it, she has been married to DH's father much longer than his mother ever was. Still, it's a sticky situation because I think his mother might be offended.

Re: Step Parents
I have a stepmother and I refer to her as my stepmother. I think its eaiser than saying my dads wife. And they have been together for like 20 years or something.
ETA: When you introdude her, you don't have to refer to her as 'mom', just as her first name, DH's stepmother.

Re: Step Parents
I agree with Farah in that you should take the cue from your DH because to me, it depends alot on the role she plays in his life. My DH has a stepfather and a stepmother and both his biological parents are alive and in his life. However, his stepfather has been around and with his mom since he was about 10 yrs old. So he introduces him as his father and we both call him by his first name.
His stepmother on the other hand, has never been involved in his life. She is introduced simply as his father's (I know, 2 fathers) wife, Donna.
He only uses the term stepfather when the 2 fathers come up and it's necessary to make the distinction.
It can be a little weird and confusing sometimes but most people get it right away and don't ask questions or make it awkward.

Re: Step Parents
I have a step-mom and we are actually pretty close. I have always called her by her first name, thus so does my DH.
Like someone else said take the cue from your spouse. My step-mom doesn't have any of her own children. She considers my sister and I her daughters, and now my DH her son-in-law.
When we have children they will call her some form of 'grandma' or 'nana'.

Re: Step Parents
I follow my DH and his sister. Both of their parents remarried. They grew up living with their mom and stepdad but spent a lot of time with their dad and stepmom, too. They call their stepdad 'Dad' but call their stepmom by her first name. So, my SIL's husband and I do the same.

I guess you would call her FIL wife or DH stepmother.
Thats kind of confusing.

That's what I do. I LOVE DH's step-mother. I just say, 'This is Brett's step-mom, Cyndi'.
ETA: I will jokingly refer to her as my 'step-mother-in-law' but only when introducing her to someone who already knows of her or that I'm close with.
