what do you need to change your last name?

SS you only need your marriage certificate and Drivers license, some will ask for your Birth Certificate~>I changed my name at SS before I changed it on my DL so your name does not need to match.

DL you need your old license, a bill proving address, marriage certificate, SS Card~>again name does not have to match

thanks
