I went to the post office. (My local P.O. does Passports) All you have to do is give them your old one & your marriage certificate. And of course pay. Not sure the cost. (I was renewing mine.)

Changing a name is free. All you need is to do is fill out the form, mail in your marriage certificate and passport. I just did mine.

it says on the passport website you have to send a certified copy of your marriage license, not a photocopy. did you just show them your certified copy or did you get a new certified copy if they keep it?

I mailed my origional & it came back in perfect condition.

I went to the Midtown office near MSG got the form and they don't accept all the original documents and forms in person. You have to mail it in to the address on the form. The application is an amendment and the cost is free since you're just changing your name. I sent mine in registered mail with a returned receipt with all the original documents: passport, marriage license, application. About a 1 1/2 months I received all the documents back in-tact. It was a matter of time.
