30th Bday Favors

Hi!
I am planning my FH's surprise 30th birthday and need to do favors, I was thinking of personalized chocolate bars. Anyone know of somewhere in Nassau County or a web site to go to for this? Someone told me Grandma's Candy Kitchen in Wantagh

Thanks!
