Addressing envlelopes

Probably a stupid question, but are you addressing wedding invitations:

Mr. and Ms. John and Mary Smith

or

Mr. and Ms. John Smith?
Also, are you saying Ms. or Mrs.

We're doing the outer envelopes as:

Mr. and Mrs. John Smith

and the inner envelopes as:

John & Mary


ETA: If they're married, it should be Mrs. and if you're just doing the outer envelopes, I think just the husband's name would be fine.

Mr. and Mrs. John Smith

inner: John and Jane Smith

Mrs. for married/widowed
Ms. single/divorced adults
Miss for girls

We're doing either:
Mr. and Mrs. John and Jane Smith
or
Mr. John and Mrs. Jane Smith.

I am very adamant about including the women's names.
