Addressing Invitations & STDs

Okay, so I know it's been asked 100 times, but here's 101!
What is the appropriate way to address the following:
- Husband & Wife (Mr. and Mrs. John Doe or Mr. John and Jane Doe)
- Husband, wife & older teenage child (Mr. and Mrs. John Doe and Jim Doe or Mr. John and Jane Doe and Jim Doe)

I'm a stickler for ettiquette so I want whichever is the most polite and proper. What did you ladies do?

I don't know if it's 100% proper, but for my STDS I did Mr. & Mrs. John Doe, and if it was a family I did Mr. & Mrs. John Doe & Family and if it was a single child I did Mr. & Mrs. John Doe (next line) John Doe Jr.
