When sending an invite to a married couple, how do I address them? Is it Mr and Mrs so and so or would I put the husbands first name on the envelope as well?

I think it's Mr. & Mrs. John Smith. Then the inner envelope would say John and Jane Smith. theknot.com has a lot of good info about addressing etiquette.
