Addressing outer envelope

I was wondering if anyone knows the proper etiquette of addressing a married couple when the woman is the doctor.
Would it read:
Mr. and Doctor John Smith?

actually, the proper etiquette is to say the female dr.'s name first, I believe.

eg... Dr. Smith and Mr. Smith

also, you would say Smith separately for both as above.
