Yes I agree. Maybe you can do a monogram on the top or something.

This was my first reaction when reading it...why would your names be on their twice?? I think the monogram would be better - JMO

Hi - no you don't use 'on' or a period. What you could do is have a little space in between the line where 'children' is and the line that shows the date.
