Another passport question...sorry, but i hope someone can help
FH legally changed his last name a few years ago, but never got a new passport with this new name. In the meantime, his passport has expired, and I'd like him to get a new passport soon.
We've looked online at the passport website, and we're not quite sure which form he has to fill out...the regular renewal form or the change-of-name form.
Does anyone know?
Thanks!!

I went to the post office and asked question before completing my renewal process of my passport I think that would be the best way to go about getting the answers to make nothing goes wrong

This is what I'd recommend doing, too.
I went to the post office in West Hempstead, they take pics there too ... one stop passport application-ing!

I actually just called the post office yesterday to set up our apt. The lady there was so helpful! She asked me a few questions about FH and I, like our age, previous marriage status, and if we ever had a passport before. She then told us what we needed to bring along, forms, money, ect.
She was soooo helpful! I'd recommend calling your Post Office and asking! They can point you in the right direction! Hopefully!

I never had a passport and i just went to the post office last week to apply for one. They told me it takes about 6-8 weeks to apply. You just go to the post office ask for the application for a passport, fill it out and bring it back with your original birth certificate and license. Oh and you need to have two pictures. You can take it there which is $15 bucks or just go to CVS its about half the price. FYI, it cost me $98 to apply for one. They also keep your birth certificate and mail it back to you with the passport.

if he changed his name legally years ago and did all the paperwork for that then when he applies he can do it with his new name. Are you going to the passport agency or the post office to do this? If you go to the passport agency they will have the forms there for you so dont worry about filling it out now. My dad had to do this. Make sure you bring all the documentation needed. Bring the papers that he changed his name.

I would call your congress person. They have a person in their office that is up on all the passport info. You can double check with the post office but I don't always trust the workers in the P.O. but that is just my opinion.

Thank you ladies for your advice...this weekend I'll have FH prepare all his paperwork and we'll take it from there in the coming week.
