I would avoid 'and guest' - I think it is so much nicer when you find out the person's name and include it...always made me feel better when I attended weddings. JMO :)

Ideally you would do #1 But I had long names to so when I knew the name of the guest I just the names with no Mr. Mrs. Honestly, noone cares.

I was at a wedding and the couple knew my name and still put and guest. I was definitely offended by that.
