anyone change their name at the socail security office in melville?
i never knew it was there till a few weeks ago but i cant find it online. Did anyone go there to change your name?? what did they make you show as ID? i found some forms online but its asking for liek 4 forms of ID and both of my parents SSNs and stuff i dont think its the right one. TIA

I went there. I think I just downloaded the forms online and had them filled out before hand. I brought my license and my marriage certificate, maybe my passport too. There was nobody in there when I went. I was in there for no more than 5 minutes.

I did. I believe I just needed the marriage certificate and the basic two forms of id... I forget exactly, but passport and birth certificate, or something along those lines, should do it, plus a checkbook for whatever the fee is. It really wasn't a big deal at all.

I think that's the one dh went to get me a new card - they didn't even check the marriage license! Just gave him the new one. I think he waited like 15 mins.

And you don't HAVE to put your parents SS on it, it says something like if you know it underneath it. I didn't put it on there, plus my parents wouldn't give me their number! Not that they don't trust me, but they don't want that number floating around out there, so are so paranoid!!!

hanks ladies! my problem is right now i can not find my passport or BC ugh!!!!! all i have is my old SSC my license and my marriage certificate.....guess ill have to wait! Thanks!
Also where exactly is it? it is announced on my bus route home but i dont know what building it is! thanks

I went to the one in Melville and I only brought my ss card, my licence and marriage certificate (and of course the form). I didn't need a passport or anything like that.
btw...I know ss offices are notorious foe being super busy but when I went, that particular office was totally dead....I did it on my lunch break, it was great!
eta: it's a grey office building, i probably wont describe this right but part of the building hangs over the parking lot. If I remember correctly, it's almost right behind 7-11 (??)
