Anyones FH/DH in the electrical union? Or just got called for physical? Major help?

Dh just got his physical form, however they will not give him any info as to whether or not to complete the yellow form.

If anyone has any information pleaes let meknow. Im sure youknow how 'serious' They are about following their rules and one mistake means you're out...
