new update on TMZ:

TMZ has learned Sean Preston and Jayden James were turned back over K-Fed tonight just before 11:00 PM PT.

We're told that while cops were at the troubled pop star's home, they discovered Spears 'under the influence of an unknown substance.'

Several ambulances have been called to the scene.

Man........thats terrible........that girl is practically SCREAMING for someone to help her. She is def. sick. I think she has all the wrong people around her and no one to really help her get better. So sad

OMG THE DRAMA! TMZ is there as Britney Spears was just wheeled out of her home on a gurney by paramedics! Why god why?!?

We're told Britney is being taken to a local hospital on a 'medical hold' -- essentially a mental evaluation. Cops found her earlier this evening at her home under the influence of an 'unknown substance.'

Cops were called to the popnightmare's home in Beverly Hills this evening after she reportedly refused to return custody of her two kids back over to their father's bodyguard. Oh the insanity of it all!

Nite Jess. Sweet Dreams

im watching the live feed and its so unbelievably sad....papz are all over the ambulance taking pics of her in it....they even chased it down the street.

she really needs to get away and get help.

Wow... I feel so bad for her kids!!!!!!!!!!

She definatly needs help..there's no denying that! This is all really crazy and really sad..it really gets to me when kids are involved...she needs to clean up her act..this is really, really OD now!

As a person working in the child protective field, if she was a regular person, those children would not have remained with her or had unsupervised visits which I assume occurred in order to let her be alone with her tonight . It's the simple fact that she is a celebrity that she is able to get away with so much. If she were one of my clients, she would have unfortunately and sadly, lost her children. My fellow caseworkers and I frequently discuss this case and say if we were involved none of this would have occured on our watch.

I grieve for these kids as they will have to go through this turmoil with their mother but I also have feelings and understand that although the mother may have issues these children need to be in the care of a caretaker who is more responsible.

Really and truly, the California Child Welfare system failed these children and I wonder if someone will answer for them having to witness this tonight.
