I think it depends on what alterations have to be done to the dress. I think the more that has to be done then maybe 2 fittings at most.
As for fittings I think 2-3 months before is fine depending when the wedding is. The more busy the shop is then the further in advance they may ask you to have them do it.

My last girl just went last night for her first fitting. If there's a lot of work to be done, they may need two. If not, they go for one and then when they go to pick it up, they make sure it fits right. HTH
