I'm buying stuff for my MOH for her bridal shower. It's a 100% Italian Catholic wedding. Does anyone know if any certain family member is supposed to buy the wedding cake topper? I want to buy it for her, but I don't want to step on toes.

No I don't think it matters who buys the cake topper.

No I don't think it matters. You are a close friend to her and wouldn't be stepping on anyone's toes. Just make sure her mom, god mother, FMIL or another BM didn't get it already. In a lot of traditional Italian families (like mine and FH) most of these things like the cake topper, unity candle, money bag, toasting flutes, ring pillow and cake cutter are given to the bride at her shower.

Yeah, this would be given to her at her shower, and it's still on her registry, so it should still be available. But I'll double check with her family!
