agiftfulheart.com is where i got ours, they were only a buck each!!! and they came out AWESOME!!! everyone raved about them and said they were the cutest theyve ever seen :) check it out!!! VERY reasonable.

yes
you can use one of theirs or there is a button to upload

the quality of my magnets is GREAT! super thick and very inexpensive with a coupon code (google)

Yes, you can upload pics. I definitely recommend vistaprint.
