Does anyone have a template to print out labels for hershey bars??? TIA

The whole thing needs to be about 5.25 inches wide by 5.5 inches high....I found this graphic that shows where the folds are, just stretch it to meet the height you need, and when your done you delete it from your template. I make them as a side business, let me know if you need any help!
