I found this in one of my wedding magazines and thought it might be helpful:

To change your Social Security card:
Dowload the SS-5 form from the SS website socialsecurity.gov/online/ss-5.html once you've filled it out, you can either mail it or turn it in directly.

To change your driver's license
Visit your local full-service Department of Motor Vehicles to fill out a change of name request form. Don't forget to bring along your current drivers license and marriage license

To change your passport
This process can take longer than others (about 6 weeks). Downlod the forms you need from http://travel.state.gov/passport/get/correcting/correcting_2654.html You'll need to send back the completed forms along with your old passport-sized photos, your marriage license and the applicable fees.

Once you have the new versions of the items above, changing your name on other documents is relatively easy. Below is a list of places you'll want to notify of your name change and documents that will need to be updated

-employer
-banks
-credit card companies
-401(k) / retirement fund
-IRA
-Mortgage
-Loans
-leases
-stocks and bonds
-doctors' offices
-insurance (health, life, auto and home)
-utilities (phone, cable, gas and electric)
-will
-club memberships
-magazine subscriptions
-voter registration

i had to go to the city today and thought since i was there that i'd go to SS office in midtown to do it, well i get there and they told me that i cant do it there since i live in queens and have to go to queens i was hoping to get the process started, everything is so confusing now. Its like ok which name do i use for things now

Its like ok which name do i use for things now

Denise they should have let you as long as you let them mail the new card to a manhattan address. That is what they told me, I went to the one on 125 St. At first they brushed me off right away but then she was like, wait, you can do it as long as you give a valid manhattan address to mail it to, so I had them mail it to my BIL. ugh, I hate when they all don't know their own rules.

yeah the guy said manhattan residents only and that i would have to go to queens and he gave me a paper with the queens address. It was the one on jamaica avenue and i said i know theres one in astoria too and he said no he said not an official one. Eh its ok, ill go tomorrow morning to the one on jamaica, it is annoying but what can you do
