consent forms?

hi girls-
as you know we are leaving for a cruise soon.
Will be leaving julianne with my sil for 4 days

want to give her a medical consent form in case she needs it for emergency care

she will also have a copy of J's birth cert and her insurance card

have any of you done this?
Do you have suggestions/ a template that you can forward me?

want to get this done today so I dont forget!

Here are my suggestions, please ladies add to it:

Write/type the letter something like this:

I 'Jane Smith' give permission for my sister in law 'Susan Smith' to consent to all medical treatment for my daughter 'Julienne Smith' in my absence from Jan x- Jan x.

Sign here and print name

Also give her your insurance card.

FYI: In an emergency situation (god forbid ) they would treat your daughter anyway, consent is not needed to treat a minor in a life threatening situation (even if the parents are against it!)

Get it notorized!!!

My aunt and uncle go away alot and always leave a form with me & DH in case something happens to their 4 kids.

I believe my Aunt got the forms from their Dr...but they do look like a legal form and they are always notorized.

By having it notorized it just proves that the signature is yours.

Better to be safe than sorry.
