I used to work with someone whose name was Carnegie, but she married a Hall .. and her name is now Carnegie-Hall. On purpose. Not as bad as those above ... but bad! Kinda funny though.

I had a students whose last name was moore-dyke. Seriously who does that knowlingly??? He just went by moore, but every now and then I would get some official paperwork with his full name. . . por child.
