DIY Card Box- any ideas how to make?

Anyone know where I can buy the boxes to make one? (Already tried Michaels and Joann's Fabric) Can't seem to find anything larger than a 10inch.

Any tips on how to put together so it looks smooth and seamless?

Also, did you use a glue gun and fabric or other material/supplies?

TIA!!! :)
