DIY water bottle template?

I have word and photoshop (new at the latter)

I'm OK without it but if I have time, maybe at the airport, I'd like to design water bottle labels for the water in our OOT baskets. I'd need about 40.

Anyone have a template? TYIA

i love the ones on this website! i started to use them for our wedding but then got too lazy :)
