Domestic Partnership question

I got the application for the domestic partnership online but I can't find instructions like who to take the completed forms to and the fee. Does anyone have this information....

When FH and I did this we had to go to the City Clerk's office in Queens - he is NYPD and we were doing it for the purpose of getting me on his health ins. I can't remember the fee off the top pf my head but I do remember that I called them to get it.

Why did yo have to go to city clerk? I guess for the NYPD insurance you have to have it done legally?

I got my FH on my health insurance by claiming Domestic Partnership but I did not have to pay a fee or go to city/town Clerk. All I did was fill out the paperwork required by the insurance company & had it noterized. I had to give them documentation showing that we lived together & showing finanancial interdependency. Teh only fee I paid was $2 to have the form noterized. The whole process was really easy for me.

I guess each insurance company has different requirements.

We had to go the city clerk b/c he is an employee of NYC and the health insurance is through the city's provider.

That would be so much easier if I could just go through his insurance to file for domestic partnership...my insurance is up in September so I need to get onto his. He works for the Postal Service so maybe they have something like that since he is a government employee. Hmmm...I have to give him some homework.

This is all I had to do also. I submitted it to my union and they forward it to the insurance company.
