Donation favors??

Donation favors??

I want to donate to the prostate cancer foundation and was thinking of doing it in honor of my dad - he's getting his prostate removed tomorrow...do i make the donation in his name or in each of the guests names? does anyone have the scroll wording for the favors?? thanks
