Let's say your RSVP count is 200 and you tell the RH it's 190...do they set up 200 places though?

as far as i know, they'll set up 190 places and if more people are there, they'll try to squeeze people here or there.
i'll be giving a count of 2 or 3 less than what i will actually have because i don't want to squeeze people TOO much.

So you tell them 190 and they set it up for 190. Who tells them we need more place settings? Does the guest have to say 'hi - I don't have a seat'?

yeah i think they have to find the maitre d or someone. or they come to you which you def dont want! my rh had me give a seperate list of all the guests in abc order with their table numbers, along with a floor plan with how many people at each table, along with all the escort cards. pretty sure they want to be exact..
