If someone is bringing a guest (like not necessarily a couple) are you/did you put them on the same card or two different cards??? and if you put them on the same card did you write 'Mr. ____ & Ms. _____' or just write their names 'John Doe & Jane Doe'???

Same card, basically the same as invitations.
Mr. & Mrs on one card
John Doe Jane Smith on one card
Mr. & Mrs. Smith and Family on one card.
