Envelope wording ?

ok i have seen this so many times but now that i am doing it i cant think properly.....
ok i have to submit the list to the calligrapher sometime last week i am a bit behind so....here is the ?

1st Question:

Outter Envelope:
Mr. & Mrs. Joe Smith
address
town, state zip

Inner Envelope:
Mr & Mrs. Joe & Jane Smith
(or)
Mr. & Mrs. Joe Smith (like the outter)

2nd Question
For Single ladies/ and BF/GF relationship is the Female addressed Ms. or Miss.????

3rd Question
Divorced Women are they addressed Ms. or Miss?

My outers are the same as yours.
My inners are first names only
Single adult women are Ms.
ALSO divorced women are Ms.

ohhhhhhhhh so i am totally off huh first names only and what happens if there is a guest just

or
Joe Smith & Guest???
