My name is Katherine but I have ALWAYS been Katie. I don't think anyone has called me Katherine in 25 years!! (Except on the first day of school every year!!)
On my invitations, I am using my formal name, but can I just use Katie on my favor tags/cocktail napkins/etc.? Or should I stick with Katherine since its a formal occasion??

YES! DH has ALWAYS been Bobby on the formal invite we did Robert, but on the napkins, matches etc...we did Bobby. He is NOT Robert...so I cant totally see where your coming from.
My cousin is Katie (really Kathleen) and the same goes for her

Dh wanted to use his naem everyone calls him,, which is a similar situation to you and it was fine!! It made him comfortable.

My name is Caroline, but mostly everyone calls me Cari. I put Caroline on my STDs and some of FH's friends were like: 'Who is Caroline? We had no idea that was her real name!!' I think I will write Caroline on all my favor tags and stuff. I think you should do whatever makes you happy. If everyone calls you Katie and Katherine would just be weird, go with Katie on your tags.

Do whatever makes you most comfortable. My name is Gabrielle but everyone calls me Gaby except family, so everything for my wedding that needs names will have Gabrielle just so the family is comfortable and i personally dont care what name is on the paper stuff!!!

I used my FH's nickname on the std and I will on everything EXCEPT the invitation.

My name is Tassamai and no one calls me that they call me Taz so on the formal invite, TY cards, STD, Photobooth pictures I used my real name, Favors I used my nickname

def go by what everyone else calls you. unless you're planning a black tie wedding but i think even then, you should still use what's most comfortable to you!

I put my full first and middle name and FH's full first name (he doesn't have a middle name) on the invite, but our nicknames on the STDs and anything else that will be personalized. We're Ali and Alex to everyone we know, and most people on my side of the family think his name is Alexander, and most people on his side of the family assume my name is Allison, neither of which is actually correct, so we're sticking with what people know.

FMIL made me put Kathryn and Joseph on our invitations but on the STD I put Joey and Katie. I'm not doing anything else where my name will be printed but if it was, I would put Katie. No one calls me Kathryn and I don't think I'd realize someone was talking to me if they called me that... Same for him... even his bosses call him Joey.... not even Joe..

use katie! same thing happened with me & my hubby...we only used Joshua on the invites...everything else we used josh!
