Girls who got/are getting married at St. Kilian

I am sending out my information for invitations and I had two questions.

First, what is the address of the church

Second, is it proper to write St. Kilian Church or St. Killian's Church?

The address is 485 Conklin St. Farmingdale, NY 11735
I wrote St. Kilian R.C. Church on the invite.
