Re: hawaii girls
some we booked online - some we booked through the concierge at the hotels. DH and I did lots of research before our first trip (in 2007) and only booked those things in advance that we knew we 'had' to do.
We didn't use our TA to book any of the activities. We did it all ourselves.

Re: hawaii girls
In addition to the suggestions already given:
Oahu:
Check out the Waikiki Beach area. Looked fun! We just didn't have the time since Oahu was only a day trip for Pearl Harbor.
Maui:
Old Lahaina Luau if you're interested in a luau. It's supposedly the most authentic. It was overall really great. Not to mention the photographers giving you a mini photo shoot. Love our pics!
On Front Street in Lahaina there are plenty of shops and restaurants. I recommend a place called Kimo's for dinner. Delicious!
ETA: We rented a jet-ski. Fun times! And they gave us about 25 pics on a disc so we have the memories.

to eat:
Puka Dogs!!! Keoki's Paradise Roy's The Beachhouse
To do: Snorkeling - there's a great spot on the side of the road by the Beachhouse Restaurant. Tunnels Beach Kapalua Trail Hike (strenuous hike but so worth it) must do a helicopter ride - it's spectacular

Maui Activities:
Road to Hana Hike Maui Blue Water Rafting (snorkerling)
For Eating:
Mama's Fish House, closer to the airport Kool Kat Cafe....literally the best burger ever! And inexpensive, its right near the pier in Lahaina so great views too. Mala - also in Lahaina
Also, we went to the Old Lahaina Luau and did not like it, it was kind of boring, I wanted a more exciting luau.
