help with program wording

I have seen postd the wording to put in program to mention a relative who is no longer with us.
i tried searching but no luck.
please help.
only 2.5 weeks to go and i still have progrms to do!!!!!!!!

are you doing a candle or a reading in the person's memory?? Maybe something like 'Candle is lit in remembrance of So and So, who holds a special place in our hearts and who we know is watching over us today'.

PS!!!!!!!! I have 5 DAYS left and didn't print my programs yet LOL!!!
