Help...changing my name!

Ok ladies, I finally plan on changing my name this Monday. I took the day off from work and I think it is about time. My friends at work keep making fun of me and say that I am still not married yet.
So, I need to know exactly step by step what I need to do, where I need to go and what paperwork I need. Give me details and specifics. I don't want to mess this up and I want to be ready to go.

I went to the DMV first and then SS in Patchogue and I grabbed the forms from online, filled them out at home, so I could be quick at both stops. It took me about 2 hours to drive to both and wait in line and get everything done.

While at the DMV, also change your title at the same time, so the car is in your new married name.
