hmm.. the image of a fan attached to your hip pops in to my head, sort of like a fan fanny pack. to keep you cool.

I'm pretty sure hip fans are the puffiness under a gown at the sides, by your hips. It's that romantic renaissance gown type of look. I had a little hip fanning done on my gown. It's great because it makes your waist look tiny.
