We made an accommodations card through Vistaprint (actually used business cards) and just put:
Accommodations
A block of rooms has been reserved for your convenience at (and then listed all hotel information).

Our venue provided us with acommodation cards and we transferred it to our own pre-cut inserts for our invitations.
